That is all we send. Read the program terms below, then fill out the form and check the consent box. The box starts unchecked, and checking it is your choice. You can submit the form without it. Consent is not a condition of purchase. We ask for your name and email so we can match you to your appointment and reach you if a text does not go through. Either way you fill it in, we keep your details on file, but we only text the number of someone who checked the consent box. ## Full program terms - **What we send:** appointment reminder text messages only: your appointment date and time, and a link to reschedule.
